The single number most correlated with whether a denial gets overturned isn’t the reason code — it’s how many days pass before someone appeals it.
- Under 10 days: recovery rates hold close to baseline.
- 10–30 days: recovery starts to erode as records go stale.
- Past 30 days: many payers’ appeal windows are already closing.
